Job Summary:

The Department of Medicine at SUNY Downstate Health Sciences University is seeking a full-time Physician Assistant 1, Renal Medicine (Nights). The successful candidate will: 

• Assist in clinical care of Renal Patient in the hemodialysis setting under the supervision of the Nephrology attending.
• Receive and return calls from the In-Patient dialysis center at Downstate Health Sciences University Hospital, Emergency Room and hospital floors regarding and medical issues in regards to Renal Patients.
• Escalate and screen clinical concerns to the Nephrology attending 
• Receive and screen consultations for Renal patient including End Stage Renal Disease patients on dialysis. 
• Schedule, plan, prioritize and organize daily dialysis schedule with the hemodialysis registered nurses and Nephrology attending.
• Obtain consent for dialysis, write initial dialysis orders for any patient being admitted to until with ESRD diagnosis. Renal attending will co-sign the notes and orders in 24-72 hours.
• Call the dialysis unit of the admitted patients to identify the primary Nephrologist for any ESRD patient. Obtain information from the dialysis unit including pertinent history including primary Renal diagnosis, hepatitis screen, dialysis orders and latest lab results including HIV results if available, medication dosage, access concerns and dry weight.
• Coordinate discharge with the dialysis unit and the community Nephrologist with communication about continuing care of the dialysis patient including any new medication that have been started.
• Identify new ESRD patients and coordinate vein mapping and access placement for such patients.
• Educates all new ESRD patients about modality of Renal Replacement Therapies including Peritoneal Dialysis and Kidney Transplantation.

Required Qualifications:

• Graduate of an AMA approved Physician Assistant Program or Baccalaureate or higher degree program that meets New York State Physician Assistant requirements.
• New York State Physician Assistant Licensure. 
• National certification in New York (or in another state with reciprocity to practice in New York).
• Preferred experience with Chronic Kidney Disease patients including Hemodialysis and patients who have revived a Renal Transplant for at least 2 years.
