Essential Duties and Responsibilities include the following:

  • To manage timelines for skilled nursing care efficiently in every aspect of the patient stay through discharge.
  • To use Physician/Specialist and Facility resources judiciously in a discerning manner.
  • To coordinate and expedite care by real time communication between all stakeholders and to strive for optimum clinical outcomes.
  • To maintain courteous and professional behavior towards patients and families, hospital staff, peers/coworkers.

AVAILABILITY

  • SNFist will accept RMG and LCH patients who are transferred from any inpatient facility or directly admitted from a physician’s office 24 hours a day, 7 days a week, 365 days a year during their coverage period. RMG will ensure that patients being transferred from the inpatient facility are medically stable for transfer to the skilled nursing facility. 
  • SNFist will ensure that mentoring responsibilities, (i.e., clinical residents, nurse practitioners, etc) do not delay the management and discharge of RMG patients.

RESPONSE TIME

  • SNFist shall return Regal Medical group and Lakeside Community Healthcare (RMG and LCH) pages within 10 minutes (subject to any emergency occupying the SNFist). If the page is not returned within 10 minutes a second page will go out and every 10 minutes thereafter. RMG and LCH’s commitment is to engage the SNFist for less than two (2) minutes on such a page request.

PATIENT ROUNDS

  • SNFist shall evaluate and write management orders on all RMG/LCH patients in the skilled nursing facility next business day for weekday admissions. SNFist shall evaluate and write management orders on all RMG/LCH patients in the skilled nursing facility within 24hrs for weekend admissions.

PHONE ROUNDS

  • For RMG and LCH patients only, and as requested by RMG and LCH Medical Director, on a daily basis 5 days a week, SNF Physician (referred to as SNFist) is to participate on telephonic physician rounds with RMG and LCH Senior Case Manager/Medical Director at a mutually agreed upon time Monday-Friday., or call Medical Director/Senior Case Manager. Time estimate is 1-2 minutes per patient.  SNFist should be prepared to discuss all patients to avoid RMG/LCH’s need to re-contact physician. Reviews should be detailed and succinct with current treatment plan or disposition plan for each patient.
  • All RMG and LCH inpatients must be seen prior to daily telephonic rounds. This avoids delays in management and discharge.

CONSULTATION AND TESTING

  • SNFist will use Specialists judiciously and in a discerning manner based on clinical need. SNFist will exercise their full ability as clinicians before accessing the services of Specialists.
  • SNFist will directly contact Specialists to discuss the case and the specific reason for consultation when needed to expedite care from a skilled nursing facility.  SNFist shall report the specialist management plans to RMG and LCH Medical/Case Mgmt. Seniors.
  • SNFist will order intake labs consisting of CBC, CMP UA C&S on newly admitted patients. Other tests and specialist consultations are ordered only if suspected outcome(s) will alter management of the patient.
  • SNFist shall assist RMG and LCH in meeting timeline goals for Preoperative patients as follows:
  • a)      SNFist or case management shall contact specialist within 4-5days of procedure.
  • b)      SNFist shall order tests 48-72hrs prior to planned procedure.
  • It is the responsibility of the SNFist to only use physician providers contracted to RMG and LCH, unless an emergency situation.  SNFist will need to discuss these cases and receive approval from Regional Medical Director.

DISCHARGE PLANNING

  • SNFist shall cooperate with and utilize (as available) the services of  the assigned RMG and LCH on-site Case Manager to coordinate the transmission of clinical information to RMG and LCH Medical Director or designee, updates in the status of the clinical management of inpatients, and the discharge needs and/or transfer requirements for acceptance of patient. 
  • For RMG and LCH patients that are hospice eligible, SNFist will notify RMG/LCH case management. RMG and LCH case management will determine which contracted hospice agency is appropriate for the patient’s specific needs and will inform the SNFist. SNFist will discuss the case with the hospice agency directly.
  • SNFist shall discuss with patient/family length of stay expectations at the time of admission. SNFist shall discuss discharge plans with patient/family at least three (3) days prior to day of discharge. For special discharge needs (home visits, tests, follow-up office visits, referrals), SNFist shall notify RMG/LCH case management at least 72 hours prior to discharge to ensure that RMG and LCH can arrange such clinical services or resource needs in a timely fashion. SNFist is to remind patient/caregiver that RMG/LCH offers a full range of post discharge services.
  • SNFist shall provide a thorough and a complete sign-out to covering physicians (weekends, and national holidays) with specific instructions for treatment and management of the inpatient.  SNFist must indicate to covering physician the discharge plan for the patient so that no discharge delays or issues with post discharge management are incurred.  SNFist is responsible for signing out patients.
  • For transfers into hospital, SNFist shall not delay implementing the clinical management of the patient by repeating radiological and laboratory tests that are deemed adequate to initiate care.  SNFist shall call hospitalist and sign out patient prior to transfer.
  • SNFist shall identify patients that are high risk for readmission and work with case management to assign these patients to RMG/LCH’s High Risk patient program.
  • For the purpose of patient domicile to skilled nursing facility transfers: SNFist, acting in conjunction with the SNF case manager, shall notify RMG/LCH Medical Director/Senior Case Manager immediately of a patient’s eligibility for transfer. SNFist shall indicate what services are required upon admission. SNFist/SNF case manager will contact PCP for physician-to-physician communication to obtain latest H&P, progress note and to discuss treatment plan.
  • SNFist shall notify RMG/LCH Case Manager and commit to writing in the clinical chart any inpatient day(s) that is deemed “aberrant” as a result of skilled nursing facility related reasons that delay the management, discharge or care of the RMG/LCH inpatient. It is the responsibility of the SNFist to be in direct communication with the Case Manager to ensure that these notes are faxed to RMG/LCH for the purposes of meeting RMG/LCH quality control requirements.
  • SNFist shall evaluate and write management orders on all RMG/LCH patients in the skilled nursing facility next business day for weekday admissions. SNFist shall evaluate and write management orders on all RMG/LCH patients in the skilled nursing facility within 24hrs for weekend admissions.
  • Discharge summary guidelines: must be dictated immediately upon discharge, dependent upon the availability of test results.

The pay range for this position at commencement of employment is expected to be between $200,000- $210,000 annually; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

 

The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

  • Graduate from accredited medical school
  • Board certified (current)/eligible in own specialty
