Physician After Hours On Call Nurse -Resource Every other weekend 2 Part-time openings 37hr or 48 hour obligation
General Summary/Responsibilities:
The Physician After Hours Nurse accepts calls and functions as the PH system representative After hours, weekends and holidays. This position requires every third week rotation including weekends and holidays. Facilitates communication to PPN physician and provider practices regarding patient questions and concerns as appropriate. The Physician After hour’s nurse provides patients with appropriate resources available with the PH community to help prevent avoidable readmissions and ED visits. (PCP office visits, urgent care) The Physician After Hours Nurse also provides support for the Premier Health MMO nurse line After hours, weekends, and holidays.
DIMENSIONS:
Handles approximately 300 or more calls per month. Manages/triages incoming phone calls for PPN physicians, providers and Premier Health MMO nurse line patients who utilize the After Hour’s on Call service.
Qualifications
Education
Minimum Level of Education Required: Associate degree
Additional requirements:
Type of degree: Associates or Bachelors
Area of study or major: Nursing
Licensure/Certification/Registration
Current Ohio RN license.
Experience
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Two to three years’ full time equivalent in home care, acute care setting, or LTAC environment.
Prior specific functional responsibilities: N/A
Preferred experience: Minimum of three years nursing experience (critical care preferred).

Organizational Standards
Responsible for physician practice after hour’s patient calls, including all documentation completion during On Call week coverage.
Documents all phone calls in EPIC, Access Data Base, Physician After Hour’s tracking log.
Responsible for continuing Physician After hour’s call competency by working one shift per month (for prn as needed for coverage.)
Demonstrates the ability to effectively communicate with both internal and external customers to ensure quality customer service.
Attends all mandatory meetings, training sessions and educational requirement needs for triage competency.
