Prime Home Health is expanding and seeking several compassionate and skilled Physical Therapists who will play a crucial role in providing comprehensive rehabilitative care to individuals in the comfort of their homes along the I-35 corridor covering northeast San Antonio, New Braunfels, San Marcos, South Austin and surrounding areas. You will work closely with patients, their families, and a multidisciplinary team to develop personalized treatment plans and help patients regain functional independence and improve their overall quality of life.
Key Responsibilities:
- Evaluation and Assessment: Conduct thorough assessments of patients' physical conditions, mobility, and functional abilities to identify impairments and develop appropriate treatment plans.
- Treatment Planning: Collaborate with patients, their families, and other healthcare professionals to establish individualized treatment goals and develop comprehensive plans that incorporate exercise programs, manual therapy, modalities, and other therapeutic interventions.
- Rehabilitation Services: Provide hands-on physical therapy interventions, including therapeutic exercises, gait training, balance training, joint mobilizations, and other techniques, to help patients improve their strength, endurance, flexibility, and overall physical function.
- Patient Education: Educate patients and their families on safe and effective therapeutic exercises, proper body mechanics, and strategies for managing pain, preventing further injury, and maintaining optimal physical health.
- Documentation and Reporting: Maintain accurate and detailed documentation of patient assessments, treatment plans, progress notes, and outcomes in accordance with regulatory and reimbursement requirements.
- Collaboration and Communication: Collaborate effectively with other healthcare professionals, such as physicians, nurses, occupational therapists, and speech therapists, to ensure coordinated care and maximize patient outcomes. Communicate regularly with the patient's primary care physician and the Home Health team to provide updates on the patient's progress.
- Safety and Compliance: Adhere to all safety protocols, infection control measures, and legal and ethical standards to ensure the well-being and safety of patients during therapy sessions.
Qualifications:
- Bachelor's or Master's or DPT degree in Physical Therapy from an accredited institution.
- Current state licensure or eligibility to practice as a Physical Therapist.
- Proficiency in documenting patient information accurately and maintaining electronic health records (WellSky/Kinnser experience a plus).
- Valid CPR BLS certification
- Previous experience in a Home Health or related setting is preferred.
- Strong knowledge of anatomy, physiology, kinesiology, and therapeutic techniques.
- Excellent interpersonal and communication skills to establish rapport with patients, families, and other healthcare professionals.
- Ability to work independently, manage time efficiently, and prioritize tasks effectively.
