Job Title:Physical Therapist, Weekend Admission Home Care - Per Diem

Position Summary:
Will evaluate Physical Therapy needs of patients in their home or other community setting, prepare the rehabilitation care plan based on the assessment, provide Physical Therapy services based on the Rehab Care Plan, reassess patient’s response to services and revise the Rehabilitation Care Plan as needed. As a full-time Physical therapist, treats patients of all ages, all types of disabilities and multiple medical problems and collaborates with all team members to maximize patient outcomes. Job requires extensive standing, walking, lifting crouching, stooping, reaching, kneeling, stair climbing, writing, computer use, driving and phone use.
Requirements:
Education: Bachelors or Master’s degree in Physical Therapy
Skills: Knowledge of home exercise programs and equipment, Medicare, Medicaid and third-party Home Care and Hospice insurance reimbursement, State, Federal and Joint Commission standards for Home Care.
Experience:Home Care experience preferred.
Certification/License: Licensed to practice as a Physical Therapist in the State of New Jersey. Must have a valid driver’s license and access to an automobile. AHA CPR/AED Certificate or BLS Certification required.
Essential Functions of the Job:
- Evaluates the patient’s level of function and physical therapy needs, determines appropriate long- and short-term goals, and develops the Rehabilitation Care Plan based on patients’ needs.
- Complete a Comprehensive Assessment as required by the Conditions of Participation for Initial or Start of Care visits which includes verification of eligibility for Home Care benefit including homebound status, review Drug Regimen, complete Admission packet including signed consents and incorporate OASIS items in that assessment.
- Create Plan of Care and other orders detailing interventions, goals and outcomes in collaboration with Patient’s Medical Doctor.
- Serve as Case Manager on cases that are deemed Therapy-Only.
- Defines care goals with patient participation.
- Establish Clinical Outcomes and pursues interventions to support outcome achievement.
- Directs and aids patients in active and passive therapeutic exercise, muscle re-education, gait training, and activities of daily living to promote independence.
- Utilizes rehabilitation modalities including heat, cold, light electrical stimulation, ultrasound and massage.
- Maintains visit frequency as ordered and notifies physician and Case Manager of any changes in visit patterns or treatment plan.
- Verbally instructs patient/family/caregivers in care and use of Assistive and Adaptive equipments including, but not limited to: Hoyer lift, wheelchair, brace, crutches, cane, prosthetic and orthotic device.
- Incorporates Patient Safety in all Assessment, instructions and training and recommend Home Modifications as identified.
- Verbally instruct and provide a customized Home Exercise Program to patient, family/caregiver and/or Home Health aide and provide educational materials as indicated.
- Communicate with Managed Care Organizations and provide needed documents for pre-authorization of services, monitor insurance authorization and request for additional visit authorizations as needed.
- Communicates patient progress, needs and concerns to Nurse Case Manager, Medical Doctor and other Team Members and document such Communication.
- Participates in staff meetings and other educational venues.
- Participate in all agency educational functions including orientation of new staff, field visits with students, and Home Health Aide monthly classes.
- Able to comprehend spoken and written English, to communicate with clarity, work independently with non-combative and consistent behavior, good assessment and documentation skills, good understanding of therapeutic techniques, ability to interact with a mixed patient/staff population.
- Able to use Computer Software System for All Patient Care Documentation and synchronize computer at least twice daily.
- Incorporates the physiological and developmental age of patients into the overall plan for the delivery of patient care.
- Maintains and respects confidentiality.
- Demonstrates compliance with the Hospital and Home Care dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
- Observes Hospital policy concerning Fire and Safety regulations, Infection Control Standards, and other rules governing conduct while at work.
- Demonstrates a commitment to own work schedule by adhering to attendance standards and obtaining coverage for cases during illness or vacations.
- Provides care and services to reflect a commitment to the HNH Mission Statement.
- Completes all tasks including Documentation and other pertinent projects in a timely manner.
- Coordinate with Assisted Living Facility staff in the provision of Physical Therapy services.
- Assist in providing Vacation Coverage for Rehab Manager including but not limited to assignment of cases to Contract Therapists.
- Pursues Evidence Based Practice standards and applies to clinical practice.
- Participates in Case Conferences, Risk Management and Performance Improvement activities. Performs additional related duties as the need arises.
