Physical Therapist

Central Health Management Inc. is seeking a Physical Therapist for a PRN opportunity working in a post-acute care facility near Portland, Maine.

Responsibilities:
- Provide physical therapy services to patients in a post-acute care setting.
- Assess and evaluate patients' conditions and develop individualized treatment plans.
- Implement treatment plans using various therapeutic techniques and modalities.
- Monitor patients' progress and adjust treatment plans as necessary.
- Educate patients and their families on exercises and activities to improve mobility and function.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.

Education and Experience:

- Masters level or higher degree in Physical Therapy.

- Strong knowledge of medical terminology, anatomy, and physiology.
- Experience working with geriatric patients is highly desirable.
- Must have an active and clear Physical Therapy license to practice in Maine.

Must be able to pass a standard background check, drug screen and be up to date on vaccinations and TB Test.

Hourly wage is based upon level of experience.
