Physical Therapist

@Home Therapy is a therapist-owned private group practice of Physical, Occupational, and Speech Therapy professionals providing adult rehabilitation in the comfort of home. We are seeking a licensed Physical Therapist who can travel to patients homes for Evaluations and Treatments. This Physical Therapist position is Per-Diem, giving you the flexibility to accept patients in your local area and see them according to your schedule. @Home Therapy provides on-boarding for new therapists as well as ongoing therapist support for clinical or documentation questions. Physical Therapists qualifications:

• Valid Physical Therapist License and Registration in New York Required
• Great Communication and Organizational Skills Required
• Familiarity with EMR software and MEdicare documentation a plus
• Home care Experience a plus

Responsibilities of the Physical Therapist include:

• Evaluate and treat patients in their homes
• Communicate with therapy coordinators
• Document your visits in the EMR in accordance with Medicare regulations and company policy
