Job Description
We've partnered with an established and growing senior care provider in Bethlehem, PA, who's seeking a PT to provide home- and clinic-based physical therapy services, per physician orders. In this role, the PT will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
This full-time opportunity offers autonomy and the ability to work a caseload at your own pace.
- Contract or direct hire
- Monday - Friday, 8:00 am - 4:30 pm
- No nights, holidays, or weekends
- Provides initial and ongoing comprehensive assessments of the client's needs.
- Establishes a treatment plan in consultation with the physician
- Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy
- Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests
- Teaches, supervises, and counsels the family and client in the physical therapy program
- Instructs client in care and use of wheelchairs, braces, crutches, canes and prosthetic and orthotic devices and provides the equipment required to carry out the plan
- Records on a timely basis all evaluation data, treatments, and client's response to therapy intervention Also records quality assurance data
- Prepares an assessment of the client's condition and periodic evaluations and summaries
- Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association
- Must be licensed or registered by the State of Pennsylvania
- Must have valid driver's license and reliable vehicle
