Physical Therapist (Pt) - Home Health

Overview

HealthPro Heritage at Home is seeking to add a full-time Physical Therapist - PT for Home Health in our Wallingford Branch. Coverage area will include Wolcott, Southington, Prospect, Cheshire, Meriden, Wallingford, Waterbury, and Naugatuck.

• Annual Expected Income Potential starts from $80,000 per year and varies depending upon the visit volume and visit type.

Responsibilities

1.  Review available resident information and evaluate the impact of this information on the assessment and treatment process.

2.  Conduct and document a thorough functional assessment of the resident, utilizing standardized procedures. Assessment of home/vocational and equipment needs.

Qualifications

1.  Bachelor/Masters of Science degree in Physical Therapy.  

2.  Current license in the state of practice.
