- Perform initial and ongoing physical therapy evaluations in the community or place of residence according to APTA standards of practice as well as state standards.
- Implement Plan of Treatment on a consistent schedule and adjust this schedule and plan according to patient progress.
- Provide therapy and treatment that is supportive and restorative to life and well-being either directly to the patient or indirectly through consultation with, delegation to, supervision of, or teaching of others.
- Evaluate mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and that are realistically attainable.
- Participate as assigned in quality assessment performance improvement initiatives, and maintains professional knowledge and experience through ongoing education and training
- Bachelor's degree in Physical Therapy required. Preferred: Master's or Doctorate degree in Physical Therapy.
- Current licensure (or provisional license as allowable by the State Physical Therapy Practice Act) with Colorado Department of Regulatory Agencies to practice Physical Therapy required
