Essential Duties
- Direct physical therapy treatment
- Provide patient, caregiver, and family counseling
- Provide patient and caregiver education
- Prepare clinical notes, and progress summaries based on the attainment of goals
- Communicate with the physician who is responsible for the Home Health plan of care to the current Home Health plan of care
- Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care
- Evaluate home environment
- Assess for muscle strength, mobility, gait, ROM-potential for rehab
- Train patient in the use of prosthetic device
- Identify patient and family/caregiver needs for other Home Health services and refers
- Participate in discharge planning for patient
- Provide in-service education programs for nursing organization personnel
- Participate in peer consultation process
- Supervise Physical Therapy Assistants
Requirements and Qualifications
- Valid Oregon Physical Therapist License
- 2+ Years of physical therapy experience
- Possesses and maintains current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
