1. General job duties. Types of procedures required.
Performs assessment and evaluation of physical and functional status of patient including musculoskeletal, neurological, cardiovascular and integumentary systems as they relate to functional mobility. Analyses evaluation results to develop care plan including short and long term goals and reassesses care plan for effectiveness. Supervises PTA, collaborates and coordinates treatment plan with care team and discharge planning,
2. Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)
We provide a laptop, Telecommunication jetpack wireless modem, PPE, Candidate will document in EPIC Remote client.
3. Required skills and minimum years of experience.
1 year Home Health Physical Therapy
4. Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)
Physical Therapist licensure, CPR certification
5. Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?) Candidate will work Monday through Friday day shift independently completing care in patients home environment, no weekends or holidays, no call.
6. Floating to other units/departments. (If so, to which units/departments?) No
7. Bed size/number of rooms for this unit/dept.
Western Home Health has an average census of 280 patients, most are receiving physical therapy.
8. Nurse to patient ratio of this unit. (nursing only)
9. Dress Code:
Professional attire of scrubs or business casual, tattoos must be covered, facial piercings must be removed, natural hair color only.
10. Tell us about the unit/department specific training and orientation program.
Physical therapist will receive 2 weeks orientation including classroom and visits with a PT preceptor.
