Physical Therapist Chandler, AZ (30 min SE of Phoenix)
We are looking for a motivated Physical Therapist to join our practice full time in Chandler, AZ to provide patients with all phases of physical therapy. Our state-of-the-art office is in Chandler, conveniently located 30 minutes SE of Phoenix. We are looking for a dedicated and motivated Physical Therapist who is passionate about improving the health and quality of life of our patients. Our ideal candidate is compassionate, a people person who is a team player- collaborating with the team to provide the highest quality care, helping our patients reach their health and wellness goals, effectively and efficiently. We are looking for a strong multi-tasker and leader, who can perform evals, is well-versed in modern modalities and rehab as well as work alongside our diverse multispecialty team.

Duties:
- Patient Consultation-review health and medical histories, exam patients, review pertinent medical records/imaging, and reason for visit
- Treating between 4 to 8 patients per shift
- Consult with referring Physicians/Healthcare team regarding diagnosis
- Develop and implement physical therapy treatment programs based on each patient's particular needs
- Co-Managing patient cases with the other doctors and staff members
- Assist patients with all phases of physical therapy treatment and techniques to include exercise instruction, manual therapy techniques, modern modalities, and activities of daily living
- Compose discharge notes for patients, refer to appropriate outside sources, as necessary
- Patient education on the benefits of PT, treatment plan maintenance, and home exercises
- Documentation/progress notes- accurate and timely records, document treatments rendered in patient charts, Record patient conditions and responses to treatments in charts after each visit
- Collaboration with the other healthcare professionals for comprehensive patient care
- Communicate effectively with both the staff and patients
- Create an excellent experience for patients through a friendly and focused attitude
Requirements:
- Graduation from an accredited Physical Therapy program
- Doctorate preferred but not required
- PT license in AZ
Schedule:
- Full time
- M, W, Th - 9-6
- T - 3-6
- Fri - 9-1
Compensation:
- $80k-$95k plus bonuses (depending on experience)
