Physical Therapist Assistant, PTA Job Description:
The Physical Therapy Assistant (PTA) works under the supervision of the Physical Therapist to implement treatment plans and assist residents in their rehabilitation journey.
Physical Therapist Assistant, PTA Responsibilities:
- Assist in Assessments: Assist the Physical Therapist in carrying out treatment plan and gathering objective data for Section GG collection during Annual, Quarterly, Falls, and Significant Change screenings.
- Treatment Support: Implement treatment plans as directed by the Physical Therapist, including exercises, modalities, and mobility training.
- Monitoring: Observe residents' performance and report any changes or concerns to the supervising Physical Therapist.
- Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service (POS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs (Net Health/PCC).
- Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring (RTM) with discharged patients to home or next level of care.
- Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff.
