$100K for PTs in Central PA? PTs Shocked at Career Path to Earning 6 Figures...
PTs are often surprised by the potential to earn 100k per year.
As an entry-level PT, the starting salary typically ranges between $70,000 and $75,000 annually. However, many overlook market forces suppressing salaries, such as declining reimbursements, particularly from federal payers.
The first mistake most PTs make is focusing solely on the starting salary without considering future growth opportunities...sacrificing future earnings for a paycheck today.
At Madden & Gilbert Physical Therapy, we've created a career ladder that has taken individuals from a physical therapy tech position through PT school to becoming a staff entry-level PT, obtaining their direct access license, advancing to a senior Physical Therapist, becoming a clinical director, and ultimately reaching equity partnership. This pathway aims to help other PTs find similar opportunities in their careers.
Madden & Gilbert PT Career Ladder:
- Entry-Level PT: the primary focus here is on learning systems through mentorship with experienced clinicians to deliver effective clinical care.
- Direct Access PT: the primary focus here is on fine tuning advanced clinical skills, and the development of other competencies including team leadership and marketing.
- Senior Team Leader: this is a senior position with experience.
- Clinical Director: The primary competencies to develop at this level are leadership, financial management, team building, marketing, and operations.
- Equity Partner: this is a team member with an ownership stake in the practice involved in the planning and growth of our practice.
In addition to short-term salary considerations, total compensation considerations include:
- Wages
- Bonuses and profit sharing
- 401k or simple IRA with potential matching (e.g., 3% at our company)

Focusing on wages alone, we analyzed our average entry-level PT salary in 2022: $63,268.08. At this level, therapists are within their first two years out of school and likely undergoing mentorship in areas such as clinical decision-making and treatment techniques. We offer a COMT (Certified Orthopedic Manual Therapist) program at this stage.
Level two sees PTs obtain their direct access license. With an established patient base and solid word-of-mouth referrals in outpatient orthopedic or vestibular settings, the average direct access licensed PT at Madden & Gilbert PT earns $70,904.75 per year (in wages).
Senior Team Leaders take on student PTs and participate in marketing efforts while maintaining an established reputation within the community. On average, they earn $77,550 annually.
Clinical Directors have increased responsibilities in personnel management, financial planning for the clinic's expansion, and operations oversight. Working an average of 45 hours per week, our clinical directors' average annual salary is $102,033.60. We offer a 12-month Clinical Director Certification program and support from experienced other successful Clinical Directors and Managers within the Company.
The pinnacle level of compensation is equity partnership (for those looking for it).
