- Implement evidence-based therapeutic interventions to improve mobility, strength, balance, coordination, and overall functional abilities.
- Monitor and track progress, adjusting treatment plans as necessary to ensure optimal outcomes.
- Educate clients and their families on proper techniques, exercises, and activities to enhance recovery and prevent further injury.
- Collaborate with other healthcare professionals, such as nurses, physicians, and occupational therapists, to provide integrated care and achieve client goals.
- Document all assessments, treatments, and progress notes accurately and in a timely manner.
- Adhere to ethical and professional standards, maintaining client confidentiality and privacy at all times.
- Stay updated with the latest advancements in physical therapy techniques, research, and regulations through continuous professional development.
Qualifications:
- Bachelor's or Master's degree in Physical Therapy from an accredited institution.
- Current license to practice as a Physical Therapist in the state.
- Proven experience in providing physical therapy services, preferably in a home care or community setting.
- Strong knowledge of musculoskeletal, neurological, and cardiopulmonary conditions and their corresponding treatment methods.
- Excellent communication and interpersonal skills, with the ability to establish rapport with clients of all ages and backgrounds.
- Demonstrated ability to work independently and as part of a multidisciplinary team.
- Proficiency in using electronic documentation systems and basic computer applications.
- Valid driver's license and reliable transportation to travel to client homes.
- CPR certification and up-to-date immunizations.
