Are you a Physical Therapist that has a desire to empower patients in their own home and guide them toward quality of life and overall safety? Are you ready to act as a patient advocate as your patient pursues their road to recovery? Connected Home Health & Hospice is seeking experienced & caring Physical Therapists to serve patients on the east side of the greater Portland, Oregon community. We are looking for candidates that are experienced, patient-centered PT who is home health savvy, OASIS-C fluent, autonomous, team/culture oriented, driven, creative, fun (!), technologically adept, quality focused, positive, and overall awesome!

Connected Home Health & Hospice is looking for a Physical Therapist who is a leader in their profession, an effective communicator, will embrace our core values, and who will share our passion and energy. Our Physical Therapists are supported by a trained Physical Therapy Assistant. Team meetings can be as easy as a phone call!
About the Opportunity
  • Provides physical therapy services to patients in their homes according to a written plan of care
  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences
  • Participates in discharge planning for patient
  • Works closely with interdisciplinary team to ensure appropriate care coordination
  • Prepares and submits a clinical progress or discharge summary based on the attainment of goals as directed by Connected Home Health policy
  • Supervises Physical Therapy Assistants according to agency policy and state regulations

Requirements
  • Degree from a baccalaureate or master's program in physical therapy approved by an accredited organization
  • Licensed to practice as a Physical Therapist within the State of Oregon
  • Two years of appropriate experience as a start of care Physical Therapist. Community/Home Health experience is preferred.
  • Demonstrates good verbal and written communication and organization skills
  • Current BLS for Healthcare Workers from the American Heart Association
  • Must be a licensed driver with an automobile that is insured in accordance with state or Agency requirements and is in good working order
