Job description

Summary
Staff Physical Therapists are responsible for providing assessment and treatment intervention. Work for the Home Physical Therapist is conducted primarily in the Veterans home setting with coverage provided in other Rehabilitation Clinics as assigned. Treatment skills include but are not limited to: modalities, manual therapy, gait and transfer training, therapeutic exercises and activities , adaptive equipment training.

The duties of this position include but are not limited to the following:
Assesses and evaluates patients referred for consultation.
Develops and initiates new treatment programs which apply current Physical Therapy practice and research findings as needed or indicated by the patient population being served. Works cooperatively with other specialties to provide seamless rehabilitation treatment for patients who may require interdisciplinary and complex care.
Read and interpret written consults/prescriptions.
Independently evaluates patients, devises objectives and goals, adapting goals, objectives and plans to meet the special needs of individual patients, unusual circumstances or complex cases.
Treats patients using standardized and recognized assessments and treatment modalities.
Documents appropriately and in a timely manner in patient's electronic medical record (CPRS). Provides patient/family education as needed.
Demonstrates skill in communicating information regarding patients to interdisciplinary team members, patients and their families as needed.
Assigns and supervises subordinate Physical Therapy staff, trainees, and volunteers in the delivery or support of treatment and education to patients and caregivers.
Promotes a safe, clean, and well-organized environment for patients and staff.
Participates in interdisciplinary meetings for special programs, patient care, and discharge planning.
Acts as a consultant/subject matter expert for PM&RS staff and other facility staff as required.
Participates in continuing education courses and seminars to remain current with advances in the field and to meet continuing education requirements of VA CT Healthcare System and Connecticut State Licensure.
Participates in PM&RS in-services and provides in-services to PM&RS and other facility staff as assigned by supervisor or Chief, PM&RS.
Actively participates in student training as required.
Schedules patient visits to ensure quality Physical Therapy treatments are delivered most efficiently and effectively.
Documents evaluations, goals, treatment plans, and rehabilitation potential, using standard format in patient's medical record as required by PM&R, Joint Commission, and VA CT Healthcare System standards.
Ensures that subordinate Physical Therapy staff and trainees maintain appropriate documentation.
Participates in Performance Improvement monitoring including the formulation of monitors, data collections, peer reviews, and preparation of reports in accordance with VA and Joint Commission guidelines.
Orders supplies to ensure that adequate supplies are always on hand and that repairs and maintenance activities are performed in a timely manner. Monitors work areas to ensure a clean safety conscious environment exists for all patients and staff.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. See Chapter 2, section D, paragraph 5a, this part.
Education and Experience: The individual must meet at least one of the requirements below:
  • Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a Physical Therapist.
  • OR
  • Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a Physical Therapist.
  • OR
  • Doctorate degree in physical therapy.

Licensure: Individuals hold a full, current, and unrestricted license to practice physical therapy in
a State, Territory or Commonwealth of the United States, or in the District of Columbia.
Grandfathering Provision: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: GS-12 Physical Therapist (Full Performance Level) Education, Experience and Licensure. In addition to the basic requirements for this occupation , candidates must possess one year of experience equivalent to the GS-11 grade level, hold a valid unrestricted state license and demonstrate all the KSAs below:
Demonstrated Knowledge Skills and Abilities:
  • Ability to make autonomous clinical decisions in a Direct Access environment. This includes ability to independently evaluate and treat patients who seek physical therapy services.
  • Skill in performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors.
  • Ability to interpret findings from examination and evaluation, select appropriate test and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations.
  • Ability to establish a diagnosis within the scope of physical therapy and identify the appropriate rehabilitation intervention, including referral to another provider for further consultation as clinically indicated.
  • Ability to determine physical therapy prognosis by incorporating examination findings with the patient's preferences in order to set clinically appropriate treatment goals, optimize outcomes and maximize functional independence.
  • Ability to independently provide clinical oversight of Physical Therapy Assistants as well as students on clinical affiliations who are in Doctoral PT Programs or PT A Programs.

Preferred Experience: Minimum 3 years experience as a Physical Therapist in the home care
setting.
