Under the supervision of the Program Director, the part time Physical Therapist will provide physical therapy services in accordance with industry practice, pursuant to Adult Day Health Care, California Code of Regulations and Title 22 regulations. The Physical Therapist works as a member of Choice in Aging’s (CiA) management team, who work to ensure we meet our goal of providing the highest quality of services which enable frail elders and adults with disabilities to meet CiA’s mission to create opportunities where people can learn, grow and age independently with dignity and community.
Essential Job Functions:
Job functions include but are not limited to the following:
- Provide an assessment and create a Physical Therapy Individual Plan of Care (IPC) for every participant; all assessment paperwork must be completed by the first date of enrollment.
- Provide reassessment and quarterly reviews of the plan and function as a member of the Interdisciplinary Team (IDT).
- Ensure Individual Plan of Care (IPC) is completed in accordance with program regulations and on a monthly basis
- Establish treatment plans based on the assessment and schedule and perform treatment.
- Update and modify treatment plans based on subsequent reevaluations or with any changes of status or condition.
- Select evaluation and treatment methods appropriate to the participant’s physical and emotional status, diagnosis, and age group to formulate an appropriate care plan.
- Provide muscle, nerve, joint and functional ability tests; treatment to relieve pain, develop or restore function; assistance to achieve and maintain maximum performance.
- Provide educational information about physical therapy, injury prevention, ergonomics and ways to a promote health to participants and staff.
- Complete new employee transfer training and provide human resources with training documentation for each newly hired employee
- Develop activities for the general maintenance programs. Review and make changes as needed.
Minimum Qualifications
- Current Physical Therapy (PT) license and at least one year's experience in a rehabilitation or home health setting within the past five years.
- Must be able to demonstrate the knowledge and skills necessary to provide care primarily to the frail and/or elderly populations including people with Alzheimer’s or dementia.
- Must communicate in English both verbally and in writing, second language a plus.
- Microsoft Office suite and health medial charting system experience is required.
- Knowledge of Title 22 regulations helpful.
- CPR Certification required.
- Must have reliable personal transportation, California Driver’s License, automobile insurance and clean DMV record.
- Fingerprint clearance required.

Hourly Rate: $52.00 - $58.00 per hour, depending on experience
