HomeFree Pharmacy Services is looking for a competitive and talented Full-Time Licensed Pharmacy Technician for our Medication Management Program. Responsibilities
- Provide in-home consultation to review medications, explain the medication management program, and handle the transfer of your prescriptions to the pharmacy.
- Record a patient’s medical history accurately.
- Observe/Document patients under treatment to identify progress, side-effects of medications etc.
- Provide follow-up calls, home visits and support to ensure patient is adhering to medication protocols as prescribed.
- Assesses and provides patient/client and family caregiver education and information pertinent to diagnosis and medication management
- Ensure quality and safe delivery of home health pharmacy services to patients/clients and families/caregivers
- Provide emotional and psychological support when needed
- Communicate with patients’ family or friends to provide advice and education regarding medication management.
Skills
- Proven experience a Certified Pharmacy Technician
- Ability to speak Spanish is a plus
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team
- Familiarity with home health safety and sanitation standards and procedures
- Understanding of confidentiality obligations and nursing best practices
- Excellent knowledge of medical and home care terminology
- Good knowledge of MS Office and data entry
- Excellent communication and interpersonal skills
- Compassionate and able to handle stress
- Certified Pharmacy Technician in North Carolina is a must
Job Type: Full-time
Pay: $19.00 per hour
