JOB SUMMARY
Under the overall supervision of the Director of Pharmacy with support from the Clinical Pharmacy Manager and Staff Pharmacists, the Pharmacy Technician - Medication History is responsible for performing technical and clerical duties essential for identification of current home medications, as well as performing other pharmacy duties as needed. The Medication History and allergies will be reviewed for accuracy and documented in the electronic health record for patients admitted through the emergency department and direct admissions. This information will be used by physicians, pharmacists, and other health care providers to determine treatment plans, and will provide the basis for medication orders during and following the hospital stay.
MINIMUM QUALIFICATIONS
Education: High school diploma or GED. Completed an approved Pharmacy Technician training program and nationally certified.
Licensure/Certification: WV Pharmacy Technician License or ability to reciprocate to West Virginia.
Experience: At least two years of experience as a Pharmacy Technician preferred. Prior experience with Medication History preferred.
PHYSICAL REQUIREMENTS & ENVIRONMENTAL CONDITIONS
Heavy Work: Lift 25 pounds manually frequently, >25 pounds lift with assistance and occasionally push 150 pounds on wheels.
