The Clinical Pharmacy Specialist-Medication Management, commonly referred to as Drug Information, is a part of the Pharmacy Shared Services team. They are responsible for ensuring the safe use of medications in the health system; managing the health system drug formulary; overseeing medication-related clinical policies and procedures, as well as ensuring the integration and consistency of local practices to such policies and procedures. They are also responsible for ensuring the cost-effectiveness and safety of medication use within the system through optimal use of data and analytics. The Pharmacy Specialist will also be precepting pharmacy students, residents, and fellows in the area of drug information, Medication Safety, and pharmacy administration. The position is based at the Centralized Pharmacy Services Center (CPSC) at the Henry Ford Health Elijah McCoy building in Detroit.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- The Pharmacy Specialist, Ambulatory Medication Safety, is a member of Pharmacy Shared Services, and reports to the Pharmacy Manager, Clinical Shared Services.
- The Pharmacy Specialist is a dedicated pharmacist supporting Medication Safety and medication policy for the ambulatory clinical delivery system:
- Monitor adverse drug events and report locally and nationally as appropriate.
- Prioritize and review ASHP and ISMP best practice guidelines & ISMP quarterly action agendas, conduct gap analyses, and develop action plans as needed for relevant areas.
- Review use of Medication Safety-related automation and technology (e.g. bar-code scanning, infusion pumps, clinical decision support, automated dispensing cabinets) and develop action plans as appropriate.
- Serve as a liaison to other ambulatory clinical committees, such as quality, policy, and safety.
- Advocate for incorporating Medication Safety into ambulatory practices’ strategic plans.
- Advise and develop medication use practice standards which improve compliance and minimize practice variability
- Oversee and assist with medication use audits to evaluate compliance with federal, state, and local regulatory standard.
- Review the results of nursing medication management surveys, coordinate medication use surveys for pharmacy areas, and provide summative reports of findings
- Trend compliance and identify opportunities for increased compliance with current standards.
- Review and ensure compliance with compounding and outsourcing regulations for Shared Services.
- Ensure compliance with REMS programs within the ambulatory clinics.
EDUCATION/EXPERIENCE REQUIRED:
- Must hold a degree from an accredited college of pharmacy and must complete a residency training program or have equivalent experience in hospital pharmacy practice.
- Minimum of 3-5 years of pharmacist experience or the equivalent training and experience at another institution, preferred.
- Must be able to serve as an institutional authority in a given specialty practice area.
- Must have demonstrable leadership abilities and exhibit teamwork when working with others.
CERTIFICATIONS/LICENSURES REQUIRED:
- Must be licensed or eligible for licensure in the State of Michigan. If eligible, licensure must be obtained within 30 days of hire; failure to obtain licensure will result in termination of the Pharmacy Specialist position.
