Pharmacist-Safe Handling
Job Description
The Pharmacists are responsible for the delivery of pharmaceutical care to patients, including processing medication and IV orders and the advancement of rational, patient-oriented therapy. They act as the primary advocate for insuring the quality and cost-effectiveness of drug use in the hospital by serving as the primary resource for drug information for Physicians, Nurses and patients.
Department: Pharmacist, Hours: mix of days/evenings, 1 weekend shift per month
Responsibilities
Assists customers with inquiries, questions and concerns., Assists in training technicians and new pharmacists in order to insure quality pharmaceutical service. Supervises and verifies the work of technicians for accuracy and completeness., Assist the pharmacy in all performance improvement and quality assurance activities such as anticoagulation monitoring, SCIP, medication reconciliation, BMV, etc., Perform daily pharmacist duties as appropriate: Interprets, prepares and checks patient medications, as ordered by physicians, accurately and in a timely manner. Accurately clarifies physician orders to insure that the prescription does not result in an improper dose or create another type of hazard if filled as prescribed. Monitors patient therapies for allergies, drug interactions and appropriateness. Accurately prepares admixtures (including IVs, irrigations, T.P.N. and cancer chemotherapeutic agents). Provides pertinent drug information to the medical and nursing staffs. Instructs and educates patients on the proper use and effects of medication. Consistently enforces the drug formulary by contacting physicians on appropriate non-formulary orders. Performs all necessary functions of the pharmacy pharmacokinetics service., Performs Antibiotic stewardship functions., Performs assigned PI/QA responsibility in a timely manner., Performs nursing station and Pyxis inspections as assigned, submitting reports in a timely manner., Provides for appropriate and secure regulation and distribution of controlled substances., Records all clinical interventions., Reduces or eliminates medication errors., Remains well informed regarding the actions of the Pharmacy & Therapeutics Committee.
