Job description

Overview

The clinical/ staff Pharmacist is available in patient care areas on a 24-hour call basis.  Maintains a complete drug profile, monitors drug therapy and relates it to clinical laboratory tests and adverse drug reactions, is available to physicians to help plan the drug regimen and assists the nurse in managing the drug portion of the patient care plans for which he/she is responsible.  In addition, is an available source of drug information reinforced by an organized drug information service. The Pharmacist is responsible for analysis, confirmation, and order entry of drug orders for patients prior to filling of orders by qualified technicians and subsequently prior to dispensing manually or through AMDS.

 

The Clinical/Staff Pharmacist reports directly to the Director of Pharmacy.

Responsibilities

1. Responsible for all aspects related to product selection, storage, compounding, administration, quality assurance and dispensing of medications to both inpatient and outpatients, and the training and supervision of Pharmacy technicians in all areas related to these activities.

 

2. The promotion of rational drug therapy by maintaining a patient profile system, establishment of dosing parameters, protecting against adverse drug reactions and interactions, advising on inappropriate concurrent prescribing and routes of administration, monitors drug therapy and relates it to clinical pathology tests, available to the physician when a drug regimen is planned, assists the nurse in managing the drug portion of patient care plans, and offers essential advice to the physician in the practical application of pharmacokinetic data, and is available for written clinical consults.

 

3. Responsible for preparation and presentation of educational material for physicians, nursing and pharmacy staffs, (inservices, newsletters, other modes of communications as indicated).

 

4. Participation in those aspects of the hospital's patient care evaluation programs that relate to drug utilization, effectiveness, and hospital accreditation.

 

5. To be able to use online drug information sources, libraries, and other search types as a means of product evaluation, therapy evaluation, and quality control for all aspects of pharmacy practice including investigation of new agents prospectively for formulary additions and as a means to delete older agents where applicable.

 

6. Other duties as assigned.

Qualifications

1. Education    a) Graduate from accredited school with a Bachelor's in Pharmaceutical Sciences Degree is required; Doctor of Pharmacy (Pharm D.) Degree 

 

2. Licensure/Certification   a.) Completion of Pharmacy internship and licensure as required by law and passing examination of the Missouri State Board of Pharmacy and becoming a registered member in good standing.   b.) BLS certification

 

3. Work Experience  a.) Hospital experience of 1-3 years preferred but not mandatory. Residency Desirable.
