Job Description
Inpatient Pharmacist needed for 12 week hospital position.
Qualified candidates must have 1 year of recent Pharmacist experience working in an inpatient pharmacy. Must be able to complete 12 week assignment in Northern Arizona. This position is likely to extend past 12 weeks if candidate is available.
Job Duties:
- Review medication orders issued by authorized prescribers to assure accuracy and determines formulas and ingredients needed.
- Review patient‘s medical record to ensure relevant and accurate information is documented including at home medication and supplements in order to identify potential drug interactions and incompatibilities, duplicate therapies, and enable rational drug therapy and better patient care.
- Prepares IV admixtures and extemporaneous parenteral products under aseptic conditions.
- Knowledge of sterile compounding (and USP 800 regulations) and non-sterile compounding procedures
- Directly supervises Pharmacy Department personnel engaged in the performance of their duties and activities.
- Communicate with physicians, nurses, patients and other healthcare professionals regarding the appropriate use of medications, drug interactions, side effects, dosage, compatibility, stability, storage, etc.
- Follow quality control, security of controlled substances, and disposal of hazardous waste procedures.
- Maintains a positive professional attitude and demonstrates good interpersonal communication skills
- Knowledge of legal dispensing practices, federal pharmacy regulations and therapeutic incompatibilities
- Knowledge of drug-drug, drug-disease state, and drug-allergy incompatibilities
- Knowledge of pharmacology, therapeutics of common disease states and treatment modalities, pathophysiology, and pharmacokinetics
- Reviews, assesses and inputs chemotherapy treatment plans for oncology patients, verifies medication orders to ensure accuracy of dosage, treatment schedule, route of administration and safe administration based on laboratory information and evidence-based practices.
- Responds to Code Blue and Level 1 traumas, provides support for dose calculation, rates of infusion, routes of administration, and mixing of emergent IV/IM/IO preparations.
- Gains competency in utilization of pharmacy software.
Schedule: Monday- Friday 3 set shift schedules available. 8-5pm, 3-midnight and mid-shift
