Salary: $70.00 per hour when needed / $700 a month for signature of pharmacy application licensure
Schedule: Only needed for application of pharmacy licensure, hourly rate is only paid out if there is a need for inspection or appearance before Indiana Board of Pharmacy for licensure issuance

• Do you have sterile compounding experience with a specialty in intrathecal paid med compounding with willingness to sign a license application for a pharmacy?

License/Certification:

• Licensed Pharmacist in State (Required)

• Sterile Compounding Experience

• Specialty in Intrathecal Paid Med Compounding

Work Location: In person/ Nearby (Evansville, Indiana area)
Responsibilities:

• Sign application for pharmacy licensure

• Needed on-site for inspections or appearance in front of the Indiana Board of Pharmacy

• Ability to sign DEA License
