```Duties```
- Conduct patient assessments to determine appropriate medication therapy
- Provide patient counseling and education on medication use and potential side effects
- Collaborate with healthcare professionals to optimize patient care and outcomes
- Perform medication reconciliation and review for accuracy and appropriateness
- Ensure compliance with legal and regulatory requirements related to medication dispensing and documentation
- Participate in utilization review activities to promote cost-effective medication use
- Maintain accurate and complete patient records
```Requirements```
- Bachelor's or Doctor of Pharmacy degree from an accredited institution
- Current state pharmacist license in good standing
- Strong knowledge of medical terminology and pharmacology principles
- Completion of a pharmacy residency program preferred
- Experience in a hospital or clinical pharmacy setting preferred
- Ability to work in a fast-paced environment, including emergency room or critical care settings
- Excellent communication and interpersonal skills for effective patient service
