Status: Non-Exempt, Part Time 20 hours/wk
Hours: 4:30pm - 9:30pm
Note: Rate of pay listed does not include any applicable shift differentials. Mileage reimbursement for home care travel. Must have reliable transportation and be comfortable by providing in-home care.

Job Summary:
Performs patient care activities and other tasks as assigned to assist the primary care, therapy, and adult day health staff. Works under the direct and indirect supervision of the Participant Care Manager but is independent in the application of knowledge and skills.
Duties will include Assists in providing and/or maintaining optimal physical and emotional comforts to participants at the day center, in the participant’s home, and community setting. Provides basic ADL nursing care to assigned participants including dressing, toileting, transferring, ambulation, and bed mobility. Documents treatments/procedures, observations, nursing care, and participant activities on appropriate forms and in the medical record as appropriate. Reporting changes in the participant’s condition to the pertinent Interdisciplinary Team member.

Required Qualifications
- High School Diploma or GED required.
- Must be 18 years old.
- CPR certification (or willing to obtain)
- Must have reliable transportation and valid driver’s license.
- Minimum of one (1) year working with the frail and elderly (Preferred)
