ADMISSION HOSPICE RN CASE MANAGER:
At TruCare Hospice and Palliative Services, we are looking for a compassionate, talented and friendly Registered Nurse that is experienced in hospice and eager to join an exciting, growing organization. Hospice Nurse Case Manager must have good vision, hearing, speech and language abilities to perform and communicate with interdisciplinary team members.
SUMMARY:
Primary functions are to administer skilled nursing care for clients of all ages in their place of residence; coordinate care with the interdisciplinary team, patient/family, and referring agency; and assume the responsibility for coordination of care, good documentation skills, attend IDT, on- call duties, and admissions.

REQUIREMENTS:
· Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
· Current State license as a Registered Nurse
· Current State Driver’s License
· Current CPR and immunization record
· Nursing skills as defined as generally accepted standards of practice
· Reliable transportation, valid and current auto liability insurance
· HospiceSoft EMR experience, preferred
· 3-5 years’ experience as a Registered Nurse in a clinical care setting, preferred
· Hospice or home health experience, preferred
· Specialized skills preferred in palliation and end-of-life

Pay: $175.00 - $200.00 per hour
