Use your excellent communication and advocacy skills to partner with patients, families, physicians and colleagues to develop, implement, and monitor a comprehensive care plan including health promotion, disease prevention, and disease management activities.

Position: Registered Nurse RN - Care Coordination
Shift:PRN - weekday and 1 weekend a month
Experience:
3 years of recent clinical experience is required; Leadership experience is preferred.
Education:
Associates Degree RN or Bachelor's degree in clinical discipline; Master's Degree in a clinical discipline preferred

Licensures/Certifications:
Active PA RN license required
