Job description

Schedule: Per diem, hours vary

Salary: $25.00 / hour

The Per Diem Professional Program staff provides individual and group support to clients in a residential or day program. Teach skills to enhance independence and quality of life. Function in a professional role in order to fulfill programmatic needs/routines.

The Per Diem Professional Program staff has a range of duties including:

  • Assist in developing, implementing, and documenting individual treatment support plans.
  • Responsible for a variety of daily tasks with clients. This includes rehabilitative interventions, behavioral interventions and monitoring, scheduling appointments, monitoring progress, providing transportation, and other related tasks.
  • Provide role model and support to clients. Assist them in becoming fully integrated into their communities.
  • Assess clients' physical and emotional status, and report changes to supervisor
  • Ensure program site safety and maintenance as necessary at all times. Oversee and participate in the daily living and cleaning of the program, inside and outside with clients.
  • Perform other related duties as required.

Requirements:
  • High school diploma or equivalent is required
  • CPR and First Aid may be required within two weeks of hire
  • MAP may be required with 150 days of hire
  • Safety Care may be required within 90 days of hire
  • Other training as assigned
  • All candidates must be able to the successfully pass a multi-state CORI, reference, education, and employment checks
