Job Summary:
We are seeking an experienced and dedicated Per DiemOccupational Therapist to join our compassionate Home Health team. The ideal candidate should possess a strong background in Home Health care, with proficiency in evaluating and implementing effective occupational therapy interventions tailored to enhance the independence and well-being of patients in their homes.

Responsibilities:

• Conduct comprehensive assessments of patients' occupational performance, identifying areas of impairment and formulating realistic therapeutic goals.
• Develop individualized treatment plans based on assessment findings, focusing on activities of daily living (ADLs), adaptive equipment, and environmental modifications to improve patient functionality.
• Provide skilled occupational therapy services to patients in accordance with physician orders, addressing issues such as fine motor skills, cognitive function, and sensory integration.
• Collaborate with interdisciplinary team members, including nurses, physical therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.
• Educate patients and their families on adaptive techniques, energy conservation, and strategies to enhance independence in daily activities.
• Instruct and supervise patients in prescribed therapeutic exercises and activities to improve motor coordination and overall functional abilities.
• Document patient progress, treatment interventions, and changes in the patient's condition accurately and timely in accordance with regulatory standards.
• Participate in case conferences and team meetings to discuss patient care plans and contribute to the overall care coordination.
• Stay abreast of current occupational therapy practices, emerging trends, and Home Health regulations to provide evidence-based and up-to-date care.
• Adhere to ethical and professional standards, ensuring patient confidentiality and fostering a supportive and caring environment.

Qualifications:

• Valid state license as an Occupational Therapist.
• Minimum of 1 years of experience in Home Health Occupational therapy.
• Proficient clinical skills with a focus on patient-centered care.
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a team.
• Understanding of Medicare Home Health regulations and documentation requirements.
• Current CPR certification.
• Reliable transportation for home visits.
