Position Summary:

Under general supervision, the EmpowHER Peer Recovery Coach provides outreach, education, peer support, and care coordination/navigation services for women with substance use disorders and risk factors for HIV. This position requires extensive travel across the greater Houston area and surrounding counties, including occasional evening and weekend hours based on participant needs.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

· Provides outreach and engagement activities.
· Facilitates initial recovery capital assessment and individualized recovery wellness plan.
· aids participant in building recovery capital and positive social connections.
· Provides individualized recovery supports, Recovery Coaching, resource linkage/navigation/care coordination, participant advocacy, recovery check-ups.· provides recovery support and wellness groups, positive social activities.
· Harm reduction and overdose prevention education.
· Maintains communication with SMH Site Project Director.
· Participates in scheduled trainings and meetings, as necessary.
· Participates in program data collection and submission as needed to meet project goals; and maintains records and completes appropriate documentation.
· Implements a process for participants that assesses their strengths, challenges, and goals – not just their substance abuse history, in multiple life domains.
· Implements a strengths-based assessment process for the participant to identify resources and goals across multiple life domains that are best suited for the participant.
· Utilizes person-centered planning principles with participants to develop strength-based recovery plans.
• Develop recovery wellness plans with participants based on the wants, needs, and interests of the participant and engages with participant on a regular basis to identify progress and address any barriers. Conducts face-to-face contact to discuss recovery wellness planning.

Education/Experience:
High School diploma or general education degree (GED); or one to three months’ experience or training; or equivalent combination of education and experience. Training in gender-specific coaching; Texas RSPS training and certification. Peer status in recovery with a minimum of one (1) year stable recovery. Annual completion of Blue Basin training in the following courses: Understanding of Motivational Enhancement Therapy; trained in Motivational Interviewing/Stages of Change; trauma, abuse and neglect, violence, Post-Traumatic Stress Disorder (PTSD), and related conditions; COPSD services; and Medicaid, TANF, and Children’s Health Insurance (CHIP) eligibility.

Certificates and Licenses:
Training in gender-specific coaching; Texas RSPS Certification; CHW preferred or may be obtained within one year of hire. Additional certification as Certified Peer Specialist for Mental Health may be assigned. If driving on SMH business, must have a valid Texas Driver’s License and provide proof of current insurance.

The Women and Children we Serve:
Santa Maria provides critical and life changing services for low-income, indigent women age 18 and above and their children from Harris and the surrounding 12 counties.
Our women face many challenges to their recovery, including histories or trauma, criminal justice and Children’s Protective Services involvement, and homelessness or housing instability. Many also have co-occurring mental and physical health issues. Their children and families struggle with these issues as well, and so together we support the entire family on their recovery journey.
