Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional home health, hospice, and Palliative Care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.
The schedule for this position is Monday-Friday 8:00am-5:00pm; weekends and holidays as scheduled.

A Successful Pediatric Palliative Care Specialist - RNhas the following:

Responsibilities:
- Provide medical and emotional support for families across the continuum of care, including during times of crisis.
- Collaborate with the Interdisciplinary Group (IDG) to assess, establish, and support the achievement of patient/family-centered care goals; to determine the patient’s level of care; and to coordinate the appropriate utilization of services.
- Provide immediate and long-term assessment and documentation of the needs of patients and families enrolled in the TK program.
- Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.
- Maintain comfort, privacy, and dignity of our patients
- Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance
Requirements:
- Registered Nurse with current NC license; BSN preferred.
- Must have reliable transportation to travel to, enter and function in the homes/facility of any patient/family in the service area to assess and provide patient care.
- Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
