**Must reside in the Webb county, TX area**

Location: Remote, member visits are within 2 hour range of employees home. Looking for candidates in Webb county area

Care Manager RN Highlights:

• The required availability for this role is 8 am to 5 pm M-F, however end time varies due to member facing visit beginning later.
• Will be completing 6 home visits weekly with members MUST be in the Webb county area
•  Remote, member visits are within 2 hour range of employees home. Looking for candidates in Webb county area
• Due to pediatric population some visits are 3pm or after, we get request for Saturday visits at times
• Benefits are available after 90 days and include health, optical, dental, life, and short-term disability insurance.
• To be eligible for benefits you must work an average of 30 hours per week for your first 6 weeks.
• A 401(k) with a company match is available to full-time workers with one year of service on one of our eligibility dates.

Care Manager RN Compensation:

• The pay rate for this position is $38-$45/hr depending on experience paid weekly plus mileage reimbursement for visits

Care Manager RN Job Responsibilities:

• Prepare for visits by gathering information on member utilizing our Electronic Medical Record system reviewing diagnosis, meds, recent authorizations.
• Completing visits that are: Initial, Change in condition, MDCP, and Level 3 reassessments
• Travel to member home for in person visit lasting 2-4 hours long. Travel can be up to 2 hours one way.
• Submission of assessment within 24 hours. Any requests for services to be initiated within 3 business days. All other documentation due within 5 business days.  After 15 business days hand off member to appropriate level to be assigned to permanent Care Manager/service coordinator.
• Complete Centene University trainings prior to due date.
• Complete any task assignments within turnaround time per work process.

• Complete 6 visits per week
• All documentation submitted within established turnaround times
• Meet quality metrics
• Meet 92% on audits
• Meet 90% on member satisfaction surveys

Care Manager RN Requirements/Education:

• Registered Nurse
• Pediatric Experience
• Case management experience
• ADN or BSN Degree
• Active in good standing RN license in the state of Texas

Care Manager RN Preferred Skills/ Experience:

• Experience with a health plan
