RESPONSIBILITIES

  • Completes MDS schedules and sends to appropriate departments
  • Completes all required MDS assessments and care plans for LTC and the Swing Bed Program in accordance with guidelines and time frames outlined by CMS, and state regulations.
  • Reviews all referrals for short term and long term placement. Coordinates with rehab to determine if potential resident is appropriate for admission.
  • Completes prior authorizations for Medicare Advantage plans and sends updates as requested.
  • Identifies potential residents that have been referred that do not meet established criteria and confer with attending physician.
  • Coordinates admission of Medicare residents after performing a pre-admission assessment.
  • Ensures initial Med A orders are entered on the day of resident’s admission.
  • Ensures physician’s certification of stays forms are signed and up to date.
  • Coordinates and schedules care plan meetings with staff, residents and families.
  • Conducts discharge planning review weekly with the IDT, resident and/or family.
  • Completes recapitulation of stay and medication reconciliation upon resident’s discharge.
  • Coordinates electronic transmission of all required data per CMS, and state regulations.
  • Participates in programs of infection control and QAPI
  • Performs COVID testing on staff and residents PRN
  • Maintains COVID testing logs PRN
  • Reports COVID findings to DPH PRN
  • Reports COVID testing to NHSN as required.
  • Maintains psychotropic drug reports and confers with physicians monthly and PRN regarding consulting pharmacist recommendations. Ensures medication justifications are up to date and GDR’s are completed as the physician allows.
  • Oversees the restorative program
  • Completes Payroll Based Journal reports quarterly.
  • Participates with committees in writing and maintaining policies and procedure manuals and nursing service forms.
  • Keeps current in latest development by attending professional seminars, institutes and reading professional journals.
  • Proficient in PDPM and completes education as directed by the Administrator.
  • Formulates and reviews care plans and assures that care plan, medication and treatment record and nurses notes comply with physician’s orders.
  • Coordinates and directs weekly Medicare meetings with all auxiliary staff.
  • Charts pertinent information about residents.
  • Identifies and helps in resolving problems with nursing service.
  • Monitors and reports to the Administrator the Quality Measures, the CASPER and 5 star reports.
  • Involve resident/family in all aspects of care and resource management.
  • Will be required to attend Departmental staff meetings.
  • Monitor PDPM report of payment by intermediaries.
  • Keeps up to date on Federal and State regulations, as well as professional standards and makes recommendations on changes in policies and procedures to administration.
  • Initiate discharge planning on admission and follow through to ensure all physical, social, and emotional needs of the resident is met and appropriate referrals are made and the resident and family are insured continuity of care.
  • Work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care.
  • Maintain confidentiality at all time in accordance with Protected Health Information (PHI)


Education/ Qualifications/Requirements:

  • Graduate of an Accredited School of Registered Nursing.
  • Holds a current State of Georgia RN License.
  • Maintain current CPR Certification
  • Required to have:
  • 30 hours by a Board approved provider or
  • Verification of competency  -@ least 500 hours practiced as evidence by employer certification 

       in order to renew and maintain current RN license

  •  Continuing education on resident’s rights quarterly


Experience:

Prior experience as an MDS Coordinator in a Long Term Care Facility or worked as a RN in a Long Term Care Facility preferred but not required.


Physical Demands:

Employee is required to both sit and stand for prolonged periods of time.  Will be required to lift up to 25 lbs. or more. .Bending, stooping, are all frequently done in a workday.  The ability to think and work under stress, to tolerate being subjected to frequent interruptions, and be will willing to work beyond normal working hours as necessary.  Requires communication ability (speak, read, comprehend, and write).  


Working Environment:

Works in a climate controlled setting. May be warm due to preference of residents in facility.


Equipment:

Ability to use Office Equipment including computer and Internet. Also must be able to use medical equipment such as wheelchairs, beds, lifts, stethoscope, thermometer, IV pumps, feeding pumps, suction machines, and/or other medical equipment associated with providing care to residents of a long term care facility.
