Schedules:

  • Monday-Friday 9am-5pm

Compensation:

Summary

The Patient Navigator acts as a member of a multidisciplinary team to assess and coordinate patients’ care among multiple service providers during inpatient and outpatient treatment. The Patient Navigator is responsible for promoting the alumni program by providing meaningful opportunities for alumni to remain active in aftercare programming, volunteerism, personal and professional development, and events. The Patient Navigator is also responsible for facilitating weekend group programming twice a month. Individuals that typically thrive in the role have a self-starter personality, excellent communication skills, and demonstrated ability to motivate.

The most successful candidates will be optimistic and realistic, direct and spacious, plus ideally robustly playful. The most successful candidates will have some capacity with being entrepreneurial, nimble and adaptable, regularly “thinking on their feet.” They’ll enjoy working independently and collaboratively, as part of a larger care team. They will be comfortable with addressing differences and conflict while simultaneously also humble and eager to receive and integrate feedback. They’ll bring great care, integrity, and customer service to all they do. They’ll be able to maintain calmness and composure during high stress situations. They’ll be passionate about the impact their work makes, encouraging and inspiring their staff of Patient Engagement Specialists to always perform to their best capacity.

Responsibilities:

  • Assessing patient out of facility biopsychosocial stressors (i.e. housing, financial, legal, medical, employment, etc.) and developing a strategy to meet these needs during and after treatment episode
  • Facilitating multiple care aspects (case coordination, information sharing, aftercare planning, court related issues)
  • Maintains a good working relationship with support networks, government resources, community resources, employers, and referral sources.
  • Completion of all necessary documentation related to FMLA and Short-Term Disability
  • Maintains patient records by reviewing and completing case notes
  • Prepares patients’ discharge by reviewing and coordinating community services necessary to maintain sobriety outside of structured setting
  • Participates in weekly multidisciplinary treatment team as part of ongoing assessment of patient needs and plan for discharge
  • Daily involvement in census management including coordinating discharge schedule and communicating to treatment team and administrative team
  • Facilitates Intro to Recovery and Life Skills groups on a rotating schedule every other weekend
  • Promote alumni services and aftercare support for graduating patients and retain current Alumni
  • Keep all Alumni records up to date including, but not limited to: contact information, enrollment worksheets, outcome measures, and other shared resources
  • Drive census to IOP and aftercare services
  • All other duties as assigned

Qualifications

  • High School Diploma or equivalent
  • 1-year prior experience working with Substance Abuse populations strongly preferred
  • Previous Case Management experience strongly preferred
  • Valid, unrestricted driver’s license
  • Clean driving record for at least 3 years
  • Must be committed to drug-free lifestyle and responsible alcohol use
  • Personal passion or interest in substance abuse issues strongly preferred
