Position Overview:

We are seeking a dedicated and compassionate Remote Monitoring Case Manager to join our team and play a crucial role in our Remote Monitoring Program. The successful candidate will be responsible for providing remote monitoring services to clients with symptoms of high blood pressure, ensuring their conditions are stable through regular communication and coordination of care. This position is ideal for individuals who are passionate about improving patient outcomes and promoting proactive healthcare.


Key Responsibilities:


Client Management:

Monitor clients remotely using designated technology platforms to track vital signs, with a specific focus on high blood pressure indicators. Conduct regular assessments of clients' health status and escalate any concerning trends or symptoms.


Coordination of Care:

Collaborate with healthcare providers, including physicians, nurses, and other allied health professionals, to ensure coordinated care for clients. Facilitate communication between clients and healthcare team members to address any emerging health issues promptly.


Monthly Contact:

Establish and maintain a monthly contact schedule with assigned clients. Conduct proactive outreach to clients to assess their well-being, provide health education, and address any questions or concerns.


Data Management:

Maintain accurate and up-to-date records of client interactions, assessments, and interventions. Utilize remote monitoring technology to collect, analyze, and interpret client data for trends and potential interventions.


Educational Support:

Provide clients with relevant information on managing high blood pressure, medication adherence, and lifestyle modifications to promote overall well-being. Address clients' questions and concerns regarding their health status and management strategies.


Collaborative Teamwork:

Participate in regular team meetings to discuss client cases, share insights, and collaborate on improving program effectiveness. Work closely with other healthcare professionals to ensure a holistic approach to client care.


Qualifications:

Knowledge of high blood pressure management and related health issues.

Excellent communication skills, both written and verbal.

Proficient in using remote monitoring technology and electronic health record systems.

Strong organizational and time-management skills.
