Job description

  • Job Summary:

Interviews patients to determine social needs that may hinder their ability to seek or obtain primary healthcare.

Utilizes survey tools to determine living situation, food availability, safety and financial strain among other possible factors identified from survey.

Collaborates with Case Managers and Social Workers in relation to high-risk patients.


Minimum Job Requirements:


Work Experience:

One year of patient care experience preferred.


License/Registration/Certification:

Certified Nursing Assistant or Certified Medical Assistant preferred.


Education and Training:

High school diploma or equivalent is required.


Skills

  • Ability to work with high patient volumes at times, must be able to be organized and utilize time management skills,
  • Ability to exercise sound judgment in interactions with patients or families.
  • Ability to communicate effectively.
  • Ability to utilize computers to collect information for data entry.
  • Ability to provide excellent customer services.
  • Ability to work with people of all social, economic, and cultural backgrounds. Culturally sensitive.
  • Knowledge of hospital services.
  • Ability to be flexible, open-minded and adaptable to change.
  • Ability to read and comprehend English at a level necessary to follow written and oral instructions and safety precautions.
  • Bilingual, English and Spanish preferred.
