Job description

The Patient Identity Integrity Coordinator is responsible for ensuring the integrity of the Enterprise Master Patient Index, Patient Identity system linkages, and medical record.
Overview: The Patient Identity Integrity Coordinator investigates and resolves patient record errors in the Enterprise Master Patient Index and EMR. This position collaborates with department leadership, IT & downstream system administrators, interface teams, patient experience department, providers and patients to facilitate accurate and timely correction of, and linkages to, patient records /clinical data to ensure integrity. This position identifies, confirms, and when appropriate, consolidates duplicate patient medical records and patient identifiers. Conversely, this position also resolves patient overlays which occur when two patients' clinical information is recorded in the same record by comparing and contrasting clinical documentation to accurately separate the information into two unique medical records. The Patient Identity Integrity Coordinator authorizes movement of clinical data to support Patient Identity governance across the health system. This position supports special projects to include database clean-ups and system conversions. This position supports quality audits, accountability reporting, process improvement, and assists in the training of other department leaders and HIM staff regarding Patient Identity practices. This position will establish linkages to patient records for patient portal enrollment, unsolicited (non-CMC) results, and Continuing Care Documents received. This position processes amendment requests and monitors timely completion and response per CMC policy and HIPAA requirements.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Investigate and resolve potential duplicates in the EPI Database and source systems. Consolidate paper, microfiche or other stand alone systems If applicable,
2. Investigate and resolve patient record overlays through processes that restore the integrity of Patient Identity and medical record throughout CMC and GOH.
3. Communicate merges and overlays to impacted downstream systems and departments. Serve as the arbitrator for all systems' Patient Identity issues.
4. Maintains a reporting system for accountability and process improvement initiatives.
5. Communicates effectively with stakeholders for purposes of operations, accountability, and process improvement.
6. Performs timely and accurate linkages to patient records for patient portal enrollment, unsolicited (non-CMC) results and Continuing Care Documents received.
7. Manages the amendment request process in keeping with required timelines and notifications.
8. Attendance is an essential job function.
9. Performs similar or related duties as assigned or requested.
WORK SCHEDULE: Monday - Friday 8:00am to 4:30pm.
Qualifications
Education:
A minimum of a high school diploma or GED is required. Associates degree in Health Information Management or Bachelors degree in Health Information Management preferred.
Experience:
2 years of HIM experience in an acute care setting or alternatively, experience in managing Patient Identity databases and data integrity is required.
Licensure/Certification:
None required. RHIT or RHIA preferred.
