Summary:
This position is responsible for investigating and resolving Patient Identity issues for the Johns Hopkins Health System. These Patient Identity issues involve patient populations across the continuum of care, to include hospitalized, ambulatory, Home Health, and International patients. The Patient Identity Associate is responsible for maintaining Patient Identity in a six million patient database. The Patient Identity Associate reviews and interprets multiple sources of information to include clinical records, demographic information, obituaries, next of kin contacts, public records, and other source systems to accurately identify patients. The Associate creates new Medical Record Numbers and validates existing Medical Record Numbers to support patient safety in multiple clinical services. The Associate will identify and refer complex discrepancies caused when two patients' information is combined under one Medical Record Number for resolution. The Associate will also refer suspected cases of identity theft to Identity Management.
The Patient Identity Associate receives and triages calls from patients, providers, insurance companies, attorneys, and clinical departments related to Patient Identity. The Patient Identity Associate reviews clinical documentation when two Medical Record Numbers exist for one patient, determines which number to retain, and merges the information timely and accurately.

Education:
Requires high school diploma or equivalent. Prefer active enrollment in an accredited Health Information Management Program.
Work Experience:
A minimum of one year experience in Health Information Management is preferred or demonstrated administrative level experience in system applications (i.e. electronic medical records, etc.). Previous experience supporting integrity of a Master Patient Index or Person Identity preferred.
