We are innovating value-based care and leveraging integrated applications, population insights and staffing expertise to ensure all patients have access to high-quality, connected care services that provide better outcomes at a reduced total cost of care.Job DescriptionGeneral Job functions:* Answers the Breast Navigation Line, providing whatever assistance is needed, and refers phone calls as appropriate. Understand the appropriate contacts for referral of the patient including when to refer to the Breast Center Nurses.

Picks up messages on the Breast Nurse Navigation line. Provides an away message for navigation during holidays.* Understands the navigation workflow and directs the patient to health care services within Summit, at outside facilities, and within the community.* Creates a folder for each new navigation patient according to established system.* Manages tickler file system on a daily basis, checking for new paths (searching multiple sources at times), appointments scheduled and chart notes. Updates tickler system accordingly.* Coordinates and manages patients across the continuum of care beginning with the diagnostic mammography/or ultrasound through the diagnosis of cancer, coordinating care with the Breast Care Center and Radiology.* Monitors usage and prepares new patient binders (Invasive) and folders (DCIS) in advance and take inventory of patient education materials.* Coordinates the availability of binders and patient education materials as well as being familiar with resources available in the community.

Order as needed.* Adds email addresses to the Accession list once patient is aware of their diagnosis as indicated by the Nurse Navigator. (SharePoint)* Ensure that all patients are added to the Discharge Excel file by the 9th of every month so that they may by surveyed by NRC (SharePoint)* Monthly, ensures that all patients who have left the system are on both the paper list and the Excel Accession Outmigration List. (SharePoint)* Weekly, Report Number of new imaging navigation patients who we navigated in diagnostics on Excel Spread Sheet, both for surgical consult and Radiology Biopsy referral.

(Teams)* Monthly, provides Breast Center Statistics for their sites, having the Nurse Navigator review before submitting.* Ensures that the paper list matches the Accession List.* Send statistics to Director monthly on the first day of the month labeled for specific site.* Monthly, ensure that Accession List is fully populated for all new patients and ongoing.* Provides assistance with Survivorship Care Plans and Treatment summaries.* Start Care Plans for all new cancer patients in Athena* Utilizes Care Plan Excel sheet to complete care plans as requested by Nurse Navigation (SharePoint)* Advises patients about resources that are available at both Summit and in the community, such as support groups, genetics, exercise programs, nutritional support, prosthesis etc.* Makes follow up phone calls to patients to check on their status as assigned by the Nurse Navigator* Submits cases for Oncotype DX testing per direction from Nurse Navigator and Surgeon* Attends Oncology events/Navigation related events as requested especially awareness events.* Attends programs in the evening/weekend on occasion, as requested,* Assists the Oncology Nurse Navigators by providing administrative support, as directed.* Order Patient Education Binders, and other materials needed for patients. Keep inventory.* Assists the Clinical Director with Data Collection including but not limited to CEU/Board Cert Spreadsheets, Care Plan Compliance studies, List of current programs, Reconstruction Rates, Current Programs list, recordkeeping for NAPBC and NQMBC and other tasks related to program operations.Accountability: The Patient Navigator reports to the Clinical Program Director of the Breast Center, working closely with the Nurse Navigators.Physical Job Requirements:* Pushing and Pulling, taking frequency into consideration* Physical strength to lift heavy objects, carts, or items, taking frequency into consideration* Physical mobility, which includes movement from place to place on the job, taking distance and speed into account* Physical agility, which includes ability to maneuver body while in place* Dexterity of hands and fingers* Endurance (e.g. continuous typing, prolonged standing/bending, walking)* Balance is maintained during climbing, bending and/or reachingEducation, Certification, Computer and Training Requirements:* Minimum of two years' experience working with patients in a clinical setting, breast health experience preferred or previous patient navigation experience.* Breast Navigator Certification by the National Consortium of Breast Centers Inc.

Experience with Microsoft office, Excel spread sheets and data management desired.* General comfort level working with various computer software systems* Annual Summit Compliance Training Mandatory* CPR Certification* High school graduateTravel : Position is based out of multiple sites.
