Patient Care Manager, Rn

- Oversee an interdisciplinary team of staff to ensure the continuity of high-quality care to home health patients in accordance with physician physician-prescribed plan of care
- Ensure compliance with all applicable state and federal laws and regulations
- Hold responsibility for the overall supervision and coordination of clinical services
- Coordinate and supervise an interdisciplinary team of staff to assure the continuity of high-quality care to home health patients assigned to the team's area
- Have a current RN licensure in the state of practice
- Have current CPR certification
- Have a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
