Patient Care Manager

Responsibilities:

The ideal candidate will serve as the designated Patient Care Manager to an assigned group of patients. The Patient Care Manager will partner with the patient and care team to support patients' health-related goals through monthly contact. Patient contact may occur by phone, electronic communication, and on-site interactions. Greater than 50% of time will be spent making outbound patient calls. Work may be on-site, hybrid, or fully remote as required to meet client needs.

Additional Responsibilities:

• Collaborate with interdisciplinary healthcare teams to ensure coordinated and comprehensive Patient Care.
• Develop and implement individualized care plans based on provider recommendations and patient-directed goals.
• Monitor and engage patients participating in remote physiological monitoring.
• Equip and educate patients with resources to achieve health goals.
• Maintain accurate and up-to-date documentation of Patient Care activities.

Qualifications:

• Licensed or certified healthcare professional
• Minimum 2 years recent clinical experience required
• Primary Care experience required
• Remote work-from-home experience preferred
• Experience with high-volume outbound calls preferred
• Ability to efficiently document care plan progress in the patient medical record required
• Ability to contribute to care coordination across multi-disciplinary teams required
• Ability to maintain confidentiality with all aspects of patient information required
• Excellent verbal and written communication skills required
• Proven self-starter with strong organizational skills required

Job Type: Full-time

Pay: $17.00 - $20.00 per hour

Expected hours: 35 – 40 per week
