SalaryMadison Memorial's Offsite Clinics is seeking a professional Patient Care Coordinator to coordinate team-based care providing health services to individuals through effective partnerships with patients, their caregivers/families, community resources, and their physician within the context of the Patient Centered Medical Home (PCMH) model. The incumbent facilitates a "shared goal model" within and across settings to achieve coordinated high quality care that is patient and family centered.
