PCC Essential Job Duties:

  • The Team Lead role will work in coordination with the Population Health leadership to ensure that the work of the Patient Care Coordination team is accomplished effectively.
  • In collaboration with the manager, identifies plans and executes activities to promote annual wellness visits, care management, and to ensure compliance according to policies and procedures.
  • Responsible for supervision of the assigned team:
    1. Provides consultation and direction to team.
    2. Reviews daily reports and assures coverage and delegation for patient needs.
  • Develops the PATIENT CARE COORDination staff through collaborative learning and performance objectives:
    1. Coaches and mentor's staff.
    2. Identifies staff development and training needs and ensures that training is obtained.
    3. Provides input on employee performance through the review of complicated work assignments.
  • Assists in hiring, orienting, coaching, and counseling staff.
  • Assists on work schedules and managing payroll.
  • Works with health system and department leadership in improving work policies and procedures, participates in quality improvement activities as appropriate, including establishment and monitoring of yearly goals.
    1. Participates in staff meetings
    2. Participates in meetings and serves on committees as needed.
    3. Works with manager to review analytic reports and outcomes to facilitate improvement.
  • Fosters preservation of patient's rights through concept of patient advocacy; possessing understanding and helping people of various backgrounds.

Education, Training & Experience

  1. Must have active IN LPN license or CMA certification 
  2. Must have graduated from an accredited practical nursing program or CMA program
  3. Must have active BLS certification
