The Access Support Network, or ASN, serves the San Luis Obispo County, Santa Barbara and Monterey County communities, assisting our neighbors, friends, and family living with HIV/AIDS, hepatitis c and people who are at risk of overdose. With the help of dedicated staff and volunteers, ASN works diligently to stabilize clients' health - financially, emotionally and physically, and all at no cost.

Working toward a healthy community, ASN also offers prevention education opportunities for schools, church groups, service clubs, and businesses. For us, ensuring that everyone understands that this is a preventable disease is paramount.

Access Support Network is looking to fill a case manager vacancy in Monterey County. Competitive compensation, excellent health insurance paid at 100% by the agency and generous time off. Bi-lingual in English and Spanish preferred. Occasional work travel required.

JOB DESCRIPTION & DESIRED QUALIFICATIONS

This position primarily works with people who are the most marginalized in our community. It requires that you feel comfortable working with people who may use drugs, have mental illness, are homeless, are LGBTQ identifying or who trade sex for money in a compassionate and affirming way. This person should be organized, a self-starter, enthusiastic and work well with others.

Hepatitis C Patient Navigator

Prepare supplies, including documentation for testing events.

Provide Rapid HIV/HCV testing in office and community for individuals.

Educate clients and the community on various harm reduction strategies, including distributing Naloxone.

Follow up on all positive results to assist clients in accessing the appropriate level of health care.

Build rapport with program participants and maintain usable ways to contact them.

Refer clients to physicians.

Data Entry for services provided to clients.

Scheduling and completing program re-assessments for clients ( Hep C).Maintain and update client census.

Actively participate in agency fundraisers and promote visibility and awareness of the mission and programs of the Access Support Network.
