JOB TITLE: Patient Access Specialist
HOURS: Hybrid 12 hour alternating schedule - 1 day per week in office
Week1: Sunday, Thursday, Friday 8am-8pm
Week2: Monday, Tuesday, Saturday 8am-8pm
*off every other weekend

POSITION SUMMARY:
Reporting to the Manager of Patient Access, the Patient Access Specialist, is committed to providing the highest quality of care as identified in the GHS Mission. Responsibilities of this position are to ensure a seamless coordination of patient care from initial referral contact through the admission process. This position will also act as the primary in-office customer service contact in the Patient Access Center via telephone with referrals for patient care from physicians, hospitals, the community, and all other referral sources.
EDUCATION AND QUALIFICATIONS:

• LPN license, Associates Degree or equivalent experience preferred
• Minimum of one (1) year of experience in a healthcare setting
• Clinical sales experience preferred
• Bilingual preferred

ESSENTIAL JOB RESPONSIBILITIES:

• Ensures accurate and timely entering of palliative patients
• Assists with ordering medications for scatter beds transitioning to home
• Sends new admits to the pharmacists
• Acts in a professional and responsible manner within and outside the company; contributes a positive image.
• Receives all incoming referrals and requests supporting documentation to facilitate the referral-to -admission process.
• Initiates, updates, and completes assigned patient referrals by documenting patient information in Kantime.
• Ensures consents are accurate and completed according to GHS standards, Florida State and Federal Regulations.
• Maintains close contact with Admission Nurses and Hospital Liaisons, informing of all new referrals and follows the progress of existing referrals.
• Arranges for DME deliveries as necessary.
• Maintains records of Admission and Referral Logs and update them on a daily basis.
• Develops Monthly Reports derived from data collected throughout the month in relation to the Patient Access Department.
• Accurately documents referrals not accepted for admission, the reason, and plan for follow up.
• Maintains a professional relationship and serves as a liaison with other disciplines.
• Attends to patient and family complaints/concerns and works diligently to address these issues in an effective, timely fashion.
• Creates and promotes initiatives which will develop and strengthen working relationships with care facility partners.
• Maintains a pro-active approach to managing potential issues.
• Works to enhance communication and takes initiative to ensure problem resolution is performed in a timely and effective manner.
• Educates the community regarding Hospice care, philosophy and concept through a case by case consultation with individuals in the community, educational in-services or seminars when requested.
• Possesses the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHS standards.
• Ensures all documentation is accurate and is compliant with GHS standards, Florida State and Federal Regulations.
• Ensures professional standards of Hospice nursing practice are maintained.
• Serves as a professional resource with assisting in the mentoring of new staff when requested.
