Join our team of compassionate caregivers at Niagara Hospice!

The Pathways RN's role is to meet the needs of patients with chronic illnesses, help to maintain them in the home setting and avoid unnecessary ED visits. The RN will help facilitate the growth of the palliative program through community education and presentations. In addition, the RN will assist with hospice operations as needed. Enjoy a 4 day per week schedule withno weekends, holidays or on-call!

Duties:

Palliative Case Management – Provides patient assessment, develop/adjust plan of care, facilitate completion of advanced directives

Diagnosing – Identification of and discrimination between physical and psychosocial signs and symptoms essential to effective execution and management of the nursing regimen. Such diagnostic privilege is distinctly different from a medical diagnosis.

Treating – Promotes execution of any prescribed medical regimen and directs orders from authorized healthcare providers.

Measuring Human Response & Patient Outcomes - Evaluate signs, symptoms and processes which denote actual or potential health problems. Monitor medical condition and update plan of care.

Health Teaching & Counseling - Educate patients/families on palliative and end of live care in a proactive manner. Provide supportive counsel to patient/families.

Required Qualifications:
Graduate of an accredited school of professional nursing
RN Licensure in New York State

Work/Skill/Experience:
Minimum of three (3) years of nursing experience desired but not mandatory.
Minimum of one (1) year of case management experience desired.
