General Description of Duties
The Part-Time Youth Program Case Manager Specialist position is a non-sworn position. The Part-Time Youth Program Case Manager performs tasks that are essential to the success the various Youth Programs facilitated by the Leon County Sheriff's Office. The primary duties of this position will be general case management work providing assessments and proper referral for services to youth based upon evaluation and assessment of needs. The employee must demonstrate the ability to develop, evaluate, and analyze program needs and data collection pertaining to youth. This position adheres to the agency's core values of honesty and integrity, accountability, teamwork, trust, and respect and commitment to excellence. To be successful in this position, the employee must demonstrate competency in agency Standard Operational Procedures and General Orders, conflict resolution, team building, application of transformational leadership, data entry and retrieval, time management, basic to intermediate computer knowledge, written and oral communications, inter-personal skills, customer service, and problem-solving.
Program Work Allocation:
35% - Worship with Me Program - Coordination
25% - Council on Status of Men and Boys Liaison
20% - Back on Track Administrative Support
10% - Other Duties
Essential Duties
- Work with high-risk and/or at-risk youth between the ages of 12-24.
- Disciplined to work autonomously.
- Connect with qualified providers for service referrals.
- Create and coordinate action plans for local initiatives.
- Maintain and track success benchmarks and outcomes.
- Maintain high levels of customer satisfaction by providing valued and timely services.
- Positively represent and promote local initiatives throughout the community.
- Collect and organize weekly, monthly, and quarterly statistical data.
- Understand and carry out verbal and written instructions.
- Research, establish, and build community partnerships.
- Performs other duties as assigned.
Eligibility Criteria
- High school diploma or GED required; AND
- Associate degree or 2 years case management experience preferred; AND
- Experience with computer systems/programs and/or databases and software to extract, analyze, and present data to include GIS tools, Crystal Reports, Excel, Word, PowerPoint, and Access preferred; AND
- Must possess a valid class E driver's license with good driving history required; AND
- Must be agreeable to work varied hours, extended shifts, and be on call as needed; AND
- Be able to demonstrate skills in data tracking and report writing preferred.
Necessary Special Requirements
- Ability to lift, carry push and/or pull a minimum of 50 pounds independently.
- Position requires contact with mentally, emotionally, and behaviorally challenged individuals.
- Some tasks require the ability to perceive and discriminate sounds and visual cues or signals.
- Tasks require the ability to communicate clearly and concisely both orally and in writing.
